We do currently offer our Long-term Care Application form in two different formats to make it as easy as possible for you: Adobe Reader PDF or Plain Text.

Please remember that in submitting your application you are under NO obligation to use our services! That will only happen once you sign a Long-term Boarding Agreement. But what it does do is to help us better plan, and be ready for you and your cats should you decide to proceed.

You may return your completed application via an e-mail attachment, fax or US mail.

If you have Adobe Reader you can use the PDF version. Simply download and print it out, fill it in by hand and then return it via email attachment, fax or mail.

If you'd rather skip the fancy fonts and colors and use your word-processor (or a text editor such as Notepad) to type in your information, you can use the TEXT version. There are TWO parts to this version. Part A is your information, Part B is your cats' information. Simply download and save BOTH Part A and Part B and then open them with any text editor or word-processor. Follow the typing instructions at the top of page and type in the information requested. Then save the files as TEXT and return them to us via an email attachment (or print them and then send them to us via fax or mail).

Remember, you need use only ONE of these two versions! You don't need both. If you're not sure which version you can use, call us or email us and we'll make sure we get you the right one!
